The bottle seems like the simplest thing you bring. It's also where most people make the same five quiet mistakes — counted down to the one that touches your lips.

The old fountain spout that everyone's bottle has touched. The push-button you press with the same hand that just gripped a barbell.
Use the bottle-filler nozzle if your gym has one — the upright kind your bottle never touches. If it's the classic spout, don't let your rim make contact with it.

A warm, wet, sealed box
Never actually washing it
It's "just water," so it never gets washed. Meanwhile it sits warm in your bag for hours, lid closed, handled with chalky hands all session.
A sealed bottle that's wet and warm is a perfect little incubator. Wash it daily with soap, and give the lid and straw a real scrub weekly. The rinse-and-go crowd is fooling themselves.

Hands off the part you drink from

Mid-session, you grab the bottle by the top — fingers right over the spout you drink from — with hands that just held the same dumbbells everyone else did.
Hold the body, not the mouthpiece. Flip-cap and straw lids help, because they keep your chalky, just-touched-the-cable-stack fingers off the one part that goes in your mouth.

The floor, then your mouth
Parking it on the gym floor
Every other mistake is recoverable. This one isn't, because it connects the dirtiest surface in the building directly to your lips.
The floor collects whatever 400 pairs of shoes dragged in today — we measured exactly how grim it gets in how dirty is a gym floor. You set the bottle down, the open spout rests an inch off that surface, and then you drink from it. Wash it all you want; you just re-contaminated the one part that matters.

After every session, ideally. A bottle that travels to the gym daily, sits warm in a bag, and gets handled with chalky hands is a moist enclosed space. Daily washing with soap, and a deeper scrub of the lid and straw weekly, keeps it sane.
The floor is one of the dirtier surfaces in the building, and the bottle's mouthpiece sits right where your lips go. Keeping the bottle off the floor and at eye level is the simplest fix.
